Before starting any scuba diving course or certification with Columbus Scuba, you’ll need to complete the PADI Medical Questionnaire. This form helps ensure you’re in good health to dive safely.

The medical questionnaire screens for conditions that may affect your diving safety. Most people answer “no” to all questions and dive without any issues. If you answer “yes” to any questions, we’ll discuss your specific situation with you.
